Description

The Department

The Human Services Department (HSD) delivers and supports services by forging long-term community partnerships; engaging with and building the capacity of the community to do its own problem-solving; and leveraging financial and volunteer resources.

The Position

The Human Services Specialist (Family Partner) will work as part of a team with counselors and case managers to provide services to parents and their children in Fremont, Newark and Union City. The incumbent will provide peer support and assist families in navigating systems and accessing needed services while promoting independence and advocacy skills through a respectful and non-judgmental manner. The Family Partner uses their own personal lived experience and ensures that the family’s perspective is represented in the provision of all services which will be conducted in the child’s home or school, at Youth and Family Services’ clinic located at the Fremont Family Resource Center or in other community settings.

Examples of Job Duties

  • Assists family with identifying need, existing strengths, and accessing services.
  • Provides families with information and referrals to needed supportive services and resources.
  • Serves as a peer role model and supports families as they navigate the systems.
  • Educates and mentors parents/caregivers, helping them to understand and advocate for their rights.
  • Increases families’ awareness about the benefits of parent/professional provider partnerships.
  • Participates in clinical team meetings while representing the parent’s perspective.

Ideal Candidate

Education

  • Completion of 60 semester units or 90 quarter units from an accredited college with coursework in social work, counseling, sociology, or human services related fields based on assignment.
  • A Bachelor’s degree or higher is desirable.

Experience

  • At least two years of direct service experience working as an employee, intern or volunteer in a social services environment serving children, youth, senior citizens, and/or families.
  • Possession of own personal lived experience as well as prior work experience as a Family Partner or Family advocate is highly desirable.

Licenses/Certificates/Special Requirements

  • This position requires the ability to travel independently within and outside City limits to fulfill the assigned duties and responsibilities, therefore a valid Class C California Driver’s License is required at time of appointment.
  • Possession of Peer Support Specialist certification is highly desirable.
  • Bilingual fluency in English and Spanish is highly desirable.

  • Ability to work a flexible schedule, which includes evenings and some weekends.
  • Bilingual fluency in English and Spanish.
  • Ability to work effectively with a multi-disciplinary team.
