MINIMUM QUALIFICATIONS

THE POSITION

Tri-City is currently hiring to fill one part-time vacancy for the position of Peer Support Specialist I (PSS) within the Children's Outpatient (COP) Department. The PSS position provides peer to peer support services in the office and/or in the field including, but not limited to outreach, support, encouragement, guidance, advocacy, and referrals to external resources and services for clients and their families; builds rapport with clients and their families through sharing stories of lived experience and recovery.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Provides resources and referrals to address basic needs and other barriers to wellness and recovery including food, housing, clothing, transportation, and other financial resources.

• Assists clients and families to understand behavioral health problems and steps to recovery.

• Provides support and guidance in navigating systems and obtaining additional support services; attends meetings and therapy sessions with clients (as needed); assists with completing applications and forms; and ensures client concerns are address and processes understood.

Under the direction and supervision of experienced police personnel, volunteers will provide critical support across various functions, complementing the efforts of sworn officers and civilian staff.

This is a volunteer position intended as an educational and community service experience. In addition to serving a support role within the Police Department, this classification is exposed to a wide variety of law enforcement duties and will have different opportunities, based on level of interest, to support community policing efforts and activities.

The Veterinary Technician will assist in the care of animal patients at the City’s animal shelter. Additional key responsibilities include use of medical care equipment, as well as moving and handling domestic animals and wildlife. The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

- Manage a caseload of 28-32 clients comprised of older individuals with complex medical conditions.
- Conduct comprehensive client assessments, and develop and monitor service plan.

Role Description

As a Clinical Psychologist for the County Adult Assistance Program(CAAP), you will conduct professional-level work in screening and assessing CalWORKS and CAAP clients to identify and document disabling mental health conditions and associated functional limitations that enable them to qualify for Social Security Benefits.

Essential Duties Include:

• Conducts screening and assessment of clients with mental health related complaints, collateral interviews with primary care and behavioral health staff, and medical records review to support clients’ application for Social Security Benefits.
• Selects, administers, scores, and interprets appropriate psychological tests of intelligence, personality, aptitude, achievement, interest and other specialized areas and conducts diagnostic clinical interviews with emotionally disturbed, mentally ill, chemically dependent, and intellectually disabled adults to assess their impairments to provide medical evidence in support of clients’ application for Social Security Benefits as needed.
• Prepares psychological reports presenting diagnostic findings, interview findings, functional assessments, clinical evaluations, and recommendations for the Clients’ SSI eligibility.
• Reviews and evaluates psychological, psychiatric and/or neuropsychological data and case material.
• Provides education, consultation, and information to community providers, and CAAP’s multi-disciplinary team on adult adjustment problems, mental illness, chemical dependence, culturally relevant services, and other topics related to SSI eligibility.
• Supervises psychology interns and subordinate clinical staff not including class 2574 Clinical Psychologist; conducts staff training programs both within the clinic or unit and for line agency personnel.
