The position description:

Tri-City Mental Health Authority is looking to fill multiple vacancies for the Bilingual Behavioral Health Specialist (BHS) for the Adult Services Department. The BHS will be performing central scheduling duties and provide behavioral health support services to individuals as a part of these departments. The BHS will provide support to the team assisting with duties related to network adequacy, data tracking,  processing service requests, screenings, and linkage to mental health services. The BHS will report to their direct supervisor.

This position is responsible for a variety of ongoing responsibilities including, but not limited to:

• Provides individual, group, family, and collateral rehabilitative support services to facilitate behavior modification including skill building, needs assessment, self-care/safety, and coping strategies and techniques; may provide case management services in the office and in the field.

• Develops, monitors, and evaluates client rehabilitative and case management goals and increases/decreases in targeted behavior according to client needs; monitors goal progress/regression.

• Provides linkages, resources, and referrals to address basic needs and other barriers to wellness and recovery including food, housing, clothing, transportation, and other financial resources.

• Encourages and supports clients to fully engage in services; provides outreach and assists clients in addressing and overcoming barriers to treatment.

• Plans and facilitates and/or co-facilitates group sessions for clients and community members focused on skill building; tracks client attendance and follows-up with individual clients, as necessary.

• Provides advocacy for clients to ensure inclusion in all aspects of the community including socialization, employment, education, and housing to meet client needs.

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.
 
The current vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive.  The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position.  The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.
 
This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:
 

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.
• Provide education, outreach, and community resources information.
• Participate in quality improvement efforts.
• Collaborate with quality assurance and program integrity to ensure proper adherence to regulations.
• Prepare for and attend Fair Hearings.
• Engage in interdisciplinary team meetings or other activities to support San Mateo County's vision for integrated services.

SALARY
Social Worker II:   $6,564.13 - $8,205.60/month

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies.  Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

The San José Fire Department is recruiting to fill (3) three Hazardous Materials Inspector II positions in the Bureau of Fire Prevention Division. The Hazardous Materials Inspector II’s may be assigned to Hazardous Materials Development Services group to perform plan reviews and inspections, or to Hazardous Materials Code Enforcement group to conduct detailed field inspections at facilities storing, handling, or using hazardous materials including highly hazardous materials unique to Silicon Valley. Both group activities do cross over, as required for back up and special demands and for cross training, as needed.

 

The Hazardous Materials II inspector inspects, reviews, and enforces commercial and industrial facilities for compliance with the San Jose Fire Code, California State Fire Marshal (CSFM) regulations, CSFM adopted sections of the California Building Code Series, San Jose Hazardous Materials Storage Permit Ordinance, S and applicable National Fire Protection Association (NFPA) nationally recognized standards.

 

Duties for this position include, but may not be limited to:

The Position

Under the direction of the Youth and Family Services Administrator, the Senior Program Coordinator plays a unique role overseeing the early childhood mental health program, providing direct counseling services and supervising several staff counselors and interns.

The Senior Program Coordinator typically has specialized experience, training and qualifications that enable the position to work effectively with children 0-8 years, parents and youth caregivers.

The Senior Program Coordinator works closely with the division’s Clinical Supervisors and Quality Assurance Specialist to ensure that clinical services comply with all policies, procedures and legal regulations, and that client services are delivered at the highest standards. The Position

The Multipurpose Senior Service Program (MSSP) Case Manager will perform professional and clinical social work tasks and duties for older adults living with complex medical conditions to assist them to obtain optimum health, financial, and/or social services. As a member of the Human Services Department, this position reports to a Clinical Supervisor in the Aging and Family Services Division.

Under guidelines provided by the State of California, the MSSP Case Manager provides case management to frail seniors who are MediCal eligible and 65 years of age and older, to support them to live in their own home. This position joins a multi-disciplinary team of a social worker and a nurse and will perform case management functions in the cities of Fremont, Newark, Union City and Hayward. This position requires driving to conduct community visits, including visits to client homes and medical facilities. Participants in the MSSP program have significant medical and/or caregiving deficits and are at risk of further deterioration and/or institutionalization. The Case Manager will evaluate clients for medical appropriateness (RN only), conduct assessments (health and/or psychosocial), formulate detailed care plans, access/advocate for services, and monitor client care. The program serves a diverse client population, and the candidate must feel comfortable working with monolingual non-English speaking clients and/or through interpreters.

This is a regular, part-time position (32 hours/week) and will receive pro-rated benefits.

The Position

The Case Manager will perform professional and clinical social work with individuals and/or families who are unhoused or housing insecure, and who may have complex social and medical needs including health care, financial assistance, employment opportunities, as well as housing assistance. As a member of the Human Services Department, this position reports to a Clinical Supervisor, under direction of the division manager of Homeless Care and Response (HCR), and is characterized by the responsibility to provide comprehensive psycho- social assessments and develop and implement care plans. These plans may focus on connecting clients  to public benefits, obtaining budgeting skills, accessing medical care, and obtaining more stable living arrangements. The position works within a team of professionals and will be required to work collaboratively with team members and community partners.  In order to perform responsibilities, the Case Manager will be required to conduct site visits in the company of another staff member in the field to locations where homeless persons may reside,  to medical facilities, and other community locations.

Under the supervision of a pharmacist, the Pharmacy Technician supports the provision of pharmaceutical care to patients served by the San Francisco Department of Public Health. Assists registered pharmacists in performing nonprofessional duties in an institutional pharmacy; receives, inspects, stores, and fills orders for pharmaceutical supplies. Under supervision, the Pharmacy Technician performs the following essential job functions:

• Accurately prepares unit dose medications, compounds a variety of pharmaceuticals under the supervision of a Pharmacist, including intravenous admixtures according to proper aseptic technique and in accordance with, professional standards and departmental policies and procedures, in order to maintain required levels of supply and to provide patients with needed medications;

1.   Experience: One (1) year (equivalent to 2,000 hours) of experience (in the last 5 years) assisting in the preparation of drug and pharmaceutical items in a licensed pharmacy;

AND

2.   License: Possession of a valid Pharmacy Technician License/Registration issued by the California State Board of Pharmacy. (Please attach a copy)

• License status must be "Current" according to search.dca.ca.gov/

Substitution: Possession of a valid Intern Pharmacist License/Registration issued by the California State Board of Pharmacy may substitute for the required experience and license.

Notes:

1. One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.
2. This experience must be gained from June 17, 2017 to current

Possible Special Conditions (some positions may require the following):

1. One (1) year of experience assisting in the preparation of drug and pharmaceutical items in a licensed inpatient pharmacy within the last five years.
2. Six (6) months of experience in database application support, operational support, software installation and maintenance, and performance tuning for multi-departmental and enterprise systems.
3. The ability to obtain and maintain security clearance with the San Francisco Sheriff’s Department.
