San Mateo County Health's Public Health, Policy and Planning Division has exciting field-based opportunities for well-qualified and motivated Nurse Practitioners to provide both primary and urgent care to adults from vulnerable populations within our community—people experiencing homelessness and/or in shelters, monolingual Spanish speakers and other new immigrants, and those without insurance or underinsured. These services are provided through various Mobile Clinic modalities and venues, including but not limited to street medicine, shelter medicine, and the mobile coach. We are looking for collaborative Nurse Practitioners to work in and lead various multidisciplinary service teams in the field to provide comprehensive, low-threshold, client-centered care and engagement.

Teams are staffed by a Nurse Practitioner (NP), Psychiatrist, Medical Services Assistant, AOD Case Manager, and Patient Services Assistants. These teams work closely with our community partners from the Human Services Agency and community-based organizations who locate and identify clients needing medical and/or mental health services, as well as provide additional health, housing, and social services in the field.

Under general supervision, Nurse Practitioners provide urgent care and primary health care services such as physical examinations, chronic disease management (e.g., diabetes and hypertension), and referrals/linkages to specialty clinics as appropriate.

Currently, Public Health Clinics has one full-time, regular, 40-hour-per-week vacancy for a bilingual Spanish Nurse Practitioner and two 40-hour-per-week, Limited-Term Nurse Practitioner vacancies, requiring flexible schedules and field-based work at different locations. Sixty-five percent of the NP’s time will be in the field, with the remainder spent in the office documenting notes, placing orders, and coordinating with other staff as needed.

• Provide low-threshold, comprehensive care for San Mateo County’s most marginalized and vulnerable populations, including people experiencing homelessness, those in shelters, migrant and immigrant farmworkers, the recently incarcerated, the uninsured, and those with financial challenges.

• Provide comprehensive and urgent care in a variety of non-traditional settings, including homeless encampments, outdoor locations, shelters, and a mobile RV clinic.

• Demonstrate the ability to work closely and collaboratively with a small team in often challenging environments (e.g., homeless encampments, shelters, extreme weather, individuals under the influence, or those with mental health challenges).

• Take stewardship of medical care for assigned geographic populations and/or shelters.

• Assess patients’ physical, emotional, and social status through medical history, physical examinations, and diagnostic screenings; evaluate clinical findings and lab results.

• Identify and manage common illnesses and other health problems.

• Refer urgent issues beyond scope to appropriate specialists.

• Collaborate with other health professionals following protocols for diagnosis and treatment.

• Build relationships with community partners to improve health outcomes and expand services.

Qualifications

Education and Experience
Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.

• Education: Completion of an advanced nursing degree from a program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), with emphasis in a Nurse Practitioner specialty area.

• Experience: Minimum of one (1) year of paid experience in an acute and/or ambulatory care setting.

The City of Berkeley invites you to apply for the position of Advanced Practice Provider in the Health, Housing & Community Services Department! In this role, you will make assessments, provide treatment based on a patient's physical and psychosocial condition, provide patient counseling and guidance in the maintenance of health and the prevention of illness; and perform other related work as required.

In the Public Health Division, the Advanced Practice Provider provides health care at the Berkeley High School Health Center (BHSHC) and the City of Berkeley's Public Health Clinic. This position has overall responsibility for the delivery of clinical care and supports other clinical staff.

This is an hourly, part-time position working up to 19 hours per week. It is not a benefited position.

MINIMUM QUALIFICATIONS

A typical way of gaining the knowledge, skills and abilities outlined above is:

• Equivalent to graduation from a Physician Assistant program from an accredited institution approved by the Physician Assistant Board of California (PAB) and one year of clinical experience in a health-related occupation; OR

San Mateo Medical Center (SMMC) is seeking an experienced Patient Services Specialist (PSS) - Extra-Help.
 
The successful hire will support the clinical team with all registration, eligibility, enrollment, authorization, other administrative tasks, and Admission, Discharge, and Transfer (ADT) activities for SMMC. The PSS serves as the ambassador for SMMC to validate information and educate our patients and guests with the financial process upon arrival to the facility. The PSS verifies all demographic information, validates eligibility, obtain authorizations, collects co-payments, assist with all payor notifications and communication, and enroll patients if needed. The hire will be expected to work holidays and rotating weekends, if needed, to support department needs.
 
The ideal candidate for this position will possess excellent communication skills, including the ability to explain complex medical terminology and medical insurance requirements to individuals with varying levels of medical knowledge. Additionally, they should have strong skills in conflict resolution and problem-solving.

San Mateo County Health- San Mateo Medical Center seeks well-qualified candidates for the position of Program Coordinator I in the Patient Access Division. The current vacancy is a full-time, limited term vacancy. The duration of the assignment will last about 6 months.

 

Under the direction of the Patient Access Manager, the Program Coordinator will provide support to over 150 Patient Services Assistants across various departments, including Emergency, Admitting, Long-Term Care, Same-Day Surgery, and will average 27 Federally Qualified Health Center (FQHC) sites with 8-16 hours of weekly program refresher training on several publicly sponsored coverage options.

 

Additionally, this position will assist the team with several EPIC post-go-live projects, and work queues as we work to revamp processes and enhance collaboration for improved workflows and revenue-generating process efficiencies.

San Mateo County Health's Aging and Adult Services Division is seeking qualified individuals for the unclassified position of In-Home Supportive Services Social Worker II/III. Social Workers provide a wide variety of welfare services in such areas as information and referral, protective services, out-of-home services, health related services, chore services, and other welfare services within the Health and the Human Services Agency.

The current unclassified vacancy is in the In-Home Supportive Services (IHSS) Program that provides in-home care for persons who cannot safely remain in their own home without such assistance. The recipients of IHSS are aged, blind and/or disabled and eligible for Medi-Cal. Disabled and/or blind minor children may also be eligible for IHSS. The Social Worker will make a home visit to determine eligibility for the program and to conduct an assessment to determine a range of service hours for particular tasks. The IHSS recipient or guardian is the IHSS employer and is responsible for hiring and supervising a provider for the care they receive. The Social Worker will travel throughout the County to conduct home visits, attend meetings and perform other duties of the position. The Social Worker will work in the San Mateo, CA Aging and Adult Services Office.

This IHSS Social Worker will conduct intake and/or on-going activities for IHSS recipients. Some of those activities include:

• Conduct initial IHSS assessments using a detailed State assessment tool.
• Complete necessary documents, paperwork, and work within different information technology systems.
• Conduct annual reassessments.
• Educate, support, and perform administrative duties for IHSS recipients and care providers to understand/comply with program regulations.
• Provide education, outreach, and community resources information.
• Participate in quality improvement efforts.

Social Workers provide highly skilled, intensive casework services to individuals and their families and provide casework consultation within the department and to outside agencies. Incumbents of positions at this level work with minimal supervision and are expected to have acquired the high degree of assessment skills required to perform care coordination and referral tasks in complex casework areas. Lead supervisory responsibilities may be assigned as circumstances dictate.

This is an unclassified position. Unclassified positions are at-will for a limited duration, typically associated with grant or other special funding. These positions are not covered by San Mateo County Civil Service Rules; and there is no formal probation period. San Mateo Medical Center is seeking an experienced Surgery Scheduling Specialist for a Full-Time, Extra-help position for the Surgery Department. The Surgery Scheduling Specialist provides technical, complex and specialized administrative support to surgeons and patients scheduled for surgery, which includes interaction with operating room staff and patients; handles multiple competing priorities; interacts with diverse customer groups; handles multiple patient-related tasks such as tracking patient financials to ensure information is correctly entered; and moves patients from the clinic to the operating room, including all pre-operative processes necessary for the patients prior to surgery, pre-op process sheets and clinic schedules, data entry of items into the operative database, financial reconciliation, and post-op order sets, and may have some responsibilities for ordering supplies or equipment.
 
The ideal candidate for this position will possess excellent communications skills, including the ability to communicate complex medical terminology to a variety of people with varying levels of medical knowledge. In addition, they would possess skills in conflict resolution, and problem solving. The ideal candidate will have the ability to work with challenging patients, and patients who may have a language or communication barrier. This position will require attention to detail, and therefore the ideal candidate will possess strong organizational skills and the ability to multi-task.
 
NOTE: The ability to speak, read and write in fluent English and Spanish is preferred for this position.

• Tracking patient financials to be sure they are correctly entered.

San Mateo County Health is seeking an experienced individual for the position of Public Health Clinics Process Improvement Manager that plans, oversees, and provides a wide variety of administrative, analytical, and work-flow support on a County-wide level; analyzes practices and procedures and makes recommendations for organizational, operational, policy, and procedural improvements; conducts needs analysis, feasibility studies, and program evaluation for assigned projects and programs; develops, summarizes, and maintains administrative and fiscal records; fosters cooperative working relationships among County departments and acts as liaison with various community, public, and regulatory agencies; and performs related work as required.

There is currently one regular vacancy in the Public Health, Policy and Planning Division, located in San Mateo, but the position will include travel/driving throughout the county to perform duties of the job.

This management position will report to the Public Health Clinics Manager, overseeing Edison Clinics and Mobile Clinics and will have broad project management responsibility for key strategic efforts affecting the Public Health clinics, and will exercise direct and general supervision over assigned professional, technical, and office support staff.
